Ana McKenna, CCPNH Program Manager (LTC, USAR)

Commissioned in the US Army in 1999 as a logistics officer, Ana has served on active duty, in the NH National Guard, and is currently still serving as a reservist out of West Hartford, Connecticut. Ana has worked for over 15 years connecting community resources to military families and assisting Service Members on the civilian side, while leading Soldiers through multiple deployments and beyond. She understands the complexities of military service and is proud to lead the CCPNH team in helping others. Ana loves traveling, reading and spending time with her family in her downtime.

Nancy Triantafyllou, Clinical Director

Nancy counts 27 years of field experience in providing counseling/mental health services in a variety of settings in the State of NH. She is a NH dually licensed clinician in Mental Health and Substance Abuse (NH-LCMHC, NH-MLADC) and a State approved clinical supervisor. Nancy employs evidence-base therapeutic approaches tailored to meet the unique needs of each client. Her core belief is to collaborate with each client to find their individual strengths and develop healthy coping strategies. Besides her years of work in the NH National Guard she has worked extensively with children and family systems in all levels of the behavioral health continuum.  When she is not working, she spends time on short and loooonger traveling adventures with her family.

Jeanine Bio

Jeanine Merrill, Intake Coordinator

Jeanine holds a bachelor’s degree in business management, and is a certified medical administrative assistant. She has 10+ years of experience in these fields. She can get you enrolled, send you electronic documents, and even enroll you in free tax prep. In her spare time, she enjoys walks on the beach, worship services, performing arts, and spending time with her family and rescued cat.

Katherine Fredette, Certified Financial Planner (CFP®)

Kathe is personal Financial Counselor for currently-serving Military and their Families enrolled with the NH Care Coordination Program. Coordinate a Volunteer Income Tax Assistance Program providing free tax preparation including e-filing as well as finance education for all connected with the NH Guard.  CCP-NH has supported my passion to empower individuals and families to make financial decisions that enable them to prosper by sharing knowledge consumers need to be smart spenders, savers and planners of their financial future.

Candice is a retired USMC Military Police Officer, now serves as a Certified Credit and Financial Counselor. With a distinguished military background and extensive expertise in financial management, Candice leverages her unique blend of discipline, strategic planning, and operational efficiency to enhance financial literacy and well-being among service members. Her commitment to excellence and deep understanding of the unique financial challenges faced by military personnel drive her efforts to improve their financial resilience and security.

Leslie Ela, Lead Care Coordinator

Leslie has worked with hundreds of service members and families since she started with CCP-NH in 2015. She has a Masters in Public Health from Thomas Jefferson University in Philadelphia. Leslie enjoys finding new resources and creative ways to help her clients, appreciating the unique strengths of each individual. Leslie loves being presented with new challenges and fine-tuning service delivery. When not at work, Leslie spends time with her husband and two young children. They like to explore, travel, eat good food, and try new things

Megan Frost, Care Coordinator

Megan has dedicated the last decade committed to serving those who serve. After receiving her Bachelor of Science in Human Services, her career has been dedicated to working within the military domain. She epitomizes compassion, resilience, and unwavering commitment to the well-being of service members and their loved ones. She works with the military audience, helping them navigate the sometimes-complex landscape of military life, offering vital support through case management, advocacy, and resource coordination. She has provided a crucial lifeline for individuals grappling with the unique challenges of military service, including deployment, combat stress, family dynamics, and reintegration into civilian life. In her free time, she enjoys sailing with her husband.

Jon Ciley, Care Coordinator

Jon is a retired Active Guard Reserve (AGR) Soldier with the NH Army National Guard. Over his career, he has held many roles of infantry, medic and Medical Readiness Sergeant creating a diverse background of the NH Army National Guard mission and the impacts on service members and their families. He has a passion to assist his military families and finding creative ways to meet an objective as a continuation of his giving back to the patriots who serve.

Antonio Matos, Care Coordinator

Antonio holds a BA in Psychology and planning on attending school in the fall for his MA. While serving in the Marine Corps he was asked what he would like to do when returning home. Antonio stated that he wanted to find a way to give back to the military community. CCP-NH has provided that opportunity. When he is not working Antonio enjoys to playing the guitar, weightlifting, and feeding ducks.
